INTRODUCTION: Ductal and lobular carcinoma in situ (DCIS and LCIS) are non-invasive carcinomas of the breast and have an important role in cancerogenesis. They lead to an increased risk of invasive breast cancer (BC) from 7 to 12 times. Their frequency depends mainly on maintaining a good screening program and varies from 0,7% (Bulgaria, 1996) to 18,2% (USA, 1998) of all breast carcinomas.

AIM: The aim of our study is to examine the clinical and morphological characteristics of cases with non-invasive BC, diagnosed and operated in the Breast Unit of the Military Medical Academy in Sofia, Bulgaria for a 3-year period.

MATERIALS: Our retrospective study consists of 29 patients with in situ carcinoma, operated for a three-year period from 2010 to 2012. These cases are 5,6% of all BC cases and 1,9% of all patients operated from different benign and malignant breast diseases for the same period.

METHODS: The patients are diagnosed through clinical examination, diagnostic imaging and are histologically proven with fine-needle aspiration biopsy.

RESULTS: 9 of the cases were preoperatively diagnosed with DCIS and the remaining 20 were diagnosed after the surgical operation. 19 of the patients were with DCIS (8 with comedo and 11 with noncomedo type), while the other 10 were diagnosed with LCIS. In 3 of the cases with DCIS we observed microinvasion.

CONCLUSION: When dealing with non-invasive carcinoma of the breast, a better outcome is achieved through quick decision-making and constant follow up of the cases. Screening programs are a must, so that we can discover cancer at an early stage. This will help us increase the ratio of discovered cases in favor of DCIS and will decrease the risk of metastasis.
