Paget’s disease of the breast is a rare type of breast cancer (1-2%) characterized by diagnostic difficulties among patients. Currently the histogenesis of the disease is under debate as the cancer may originate from the breast epidermis or the parenchyma. The aim of the present study is to analyze the clinicomorphological characteristics and differential diagnosis of patients with Paget’s disease treated at the Thoracic Surgery Clinic of the Military Medical Academy in Sofia, Bulgaria for a 3-year period (2010-2012). The subjects of this prospective study are 5 patients diagnosed with Paget’s disease of the breast. These patients represent 0.8% of all breast cancer patients operated during the period of interest. The clinical diagnosis is based chiefly on the erythemo-squamous lesion of the nipple, which in some cases spreads to the areola and usually precedes a tumor formation. The differential diagnosis of the disease includes eczema, Bowen’s disease and basocellular carcinoma. In 3 of the patients the diagnosis was confirmed by mammography. Exfolliative cytology may also be useful for diagnostic purposes, nevertheless surgical biopsy remains the standard for diagnosis. Two of the patients with non-invasive carcinoma of the breast underwent an organ-sparing operation, whereas the remaining 3 underwent a mastectomy followed by the relevant adjuvant therapy according to the cancer stage.
